A Report to the Registry shall:
- Be made as SOON as is reasonably possible
- Be in such form as may be prescribed
- Contain ALL the MATERIAL FACTS giving rise to the suspicion that the child has been, is being, or is likely to be abandoned, neglected, physically or sexually ill-treated, or is otherwise in need of care and protection.
